On January 9, 2026, Licensing Program Analyst (LPA) Darnella Barnes conducted an unannounced annual inspection of the child care facility. LPA explained the purpose of the visit to Licensee Nadia Safi who granted access and received the Entrance Checklist (LIC 126). At the visit were Licensee, staff Masooda Sahak and 3 children. The facility operated Monday through Friday 7:30 am – 5:30pm. There were missing required postings ,therefore a Citation B is on attached LIC809D. Records showed the last fire and disaster drill took place on 12/7/25 LPA reminded the licensee of the six-month drill requirement and the need for proper documentation. LPA reviewed capacity and ratio rules, supervision requirements, mandated reporter duties, and the non-waivable rights of children. LPA toured the home. Off-limits areas included downstairs closet, attached garage and all of upstairs,(gate at bottom of stairway) The home was clean, well-ventilated, and furnished with safe, age-appropriate toys and equipment. A fully charged 3A-40B-C fire extinguisher was available. Smoke and carbon monoxide detectors were tested and found to be working. A functioning telephone was also present. Hazardous items, including cleaning supplies, detergents, medications, and poisons, were stored out of children’s reach. No dangerous conditions were observed. The licensee stated there were no firearms in the home, and smoking was prohibited. -----CONTINUED NEXT PAGES ----- There is no backyard or designated outdoor play area observed. The facility was located across the street from a pubic park which was reportedly used for children’s outdoor playtime. LPA reminded the licensee to notify Licensing before making changes to the home or yard. Licensee CPR/First Aid certification expires 10/29/27, mandated reporter dated 8/15/25, Staff CPR expires : 10/1/27, Mandated reporter expires 9/28/27, Staff is missing pertussis and measles immunizations and therefore a Citation B is on the attached LIC809D Five children’s files were reviewed and were completed with the exception that LIC282 Affidavit Regarding Liability Insurance For Family Child Care home form was missing and Licensee was not able to provide proof of coverage insurance for the child care. Therefore a Citation B is on the attached LIC809D LPA reminded the licensee that if transportation is provided in the future, a valid driver’s license is required, children may not be left unattended in vehicles, and proper car seats or booster seats must be used. Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. -----CONTINUED NEXT PAGE ----- Incidental Medical Services (IMS) policy was discussed. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
