Exit interview was conducted and report was reviewed with Coordinator, Nitaya Robinson & Site Supervisor, Ashley Reyes . Appeal Rights were given. A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.

    101238 Buildings and Grounds(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.(1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement is not met as evidenced by: Based on observation classroom 10 had mouse glue traps with mice droppings. Based on information gathered mice droppings were also discovered on the floor and mice were seen in classroom. This poses a potential health and safety risk to children in care.
