What the inspector wrote

On October 22, 2025, Licensing Program Analyst (LPA) Darnella Barnes conducted a pre-licensing inspection at the applicant's facility to ensure compliance with Title 22 of the California Code of Regulations. The purpose of the visit was explained, and access was granted by the applicant Spogmay Sharifzad Present during the inspection were the applicant, Spouse Azizullah Sharifzad, sons Lmar Sharifzad and Wahidullah Sharifzad. Proof of property control has been verified. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The applicant hours will be Monday through Friday, from 7:30 AM to 5:00 PM, providing care for children ages infant through 10 years.. LPA advised the applicant that if liability insurance is not provided or the daycare is not named in the policy, parents must sign the Affidavit Regarding Liability Insurance (LIC 282) and file it in each child's record. -----CONTINUED ON NEXT PAGES----- The applicant confirmed understanding that the licensee must be present in the home and ensure children are supervised at all times. The applicant acknowledged that, if temporarily absent, the licensee must arrange for a qualified substitute, and that such absences may not exceed 20% of the facility\342\200\231s operating hours per day LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant. LPA conducted a walkthrough of the indoor areas. Meals, snacks, and drinks will be prepared by the applicant . The home was clean, safe, and well-ventilated. Off-limit areas include all of upstairs, kitchen and attached garage. LPA observed a functioning smoke detector, a carbon monoxide detector and a working telephone were present. The applicant was advised to complete annual maintenance on the fire extinguisher. The applicant was provided with a fire/emergency disaster drill log and advised that drills should be conducted every six months. . Applicant was reminded that no smoking is allowed... No fireplaces or open-faced heaters were observed. Applicant confirmed that no firearms or ammunition were stored in the home. The applicant understood the procedure for separating ill children.. The home was free from observable defects or conditions that could endanger a child. The outdoor area was securely fenced and locked. Off-limit areas include both sides of the home. No bodies of water were observed. No hazardous materials were accessible. The applicant was reminded that if she chooses to transport children, a valid driver\342\200\231s license is required. Children cannot be left unattended in vehicles, car seats are for transportation only, and adequate booster seats and car seats must be available for safe transport. -----CONTINUED ON NEXT PAGES ---- LPA informed the applicant that if cribs or play yards are used for infants, there must be one for each infant unable to climb out. These must allow clear access for entry and exit. Mattresses must be firm, properly fitted, and covered with fitted sheets. Cribs and play yards must remain free of loose items, bumper pads, and any objects hanging or attached to the sides. The licensee was reminded that baby walkers, bouncers, and jumpers are not permitted. Infants may not be swaddled while in care. Safe Sleep guidelines were reviewed with the licensee, and information was provided regarding the required Individual Infant Sleeping Plan (LIC 9227) for each child under 12 months, with documented nap checks until age two. LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and- resources/safe-sleep , as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was provided with information that children's personal rights cannot be waived, even with authorization from a representative. These rights include being treated with dignity, receiving safe and healthful accommodations, and being free from corporal or unusual punishment. The applicant was provided information on her reporting requirements, including a copy of the Unusual Incident Form (LIC 624B), which outlines how, what, where, and when to report incidents that threaten the physical or emotional health and safety of any child. The applicant will wait until facility opens to determine IMS needs. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP . On this date, 10/22/2025, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. The applicant was informed that a Large Family Child Care Home License would be issued upon approval of licensing program manager Exit interview conducted and report was reviewed with the Applicant, Spogmay Sharifzad. ----END OF REPORT -----
