What the inspector wrote

On 07/07/2023 at 10:00 AM, Licensing Program Analyst (LPA) Susy Cervantes met with applicant, Blanca Sabalsa, to conduct a pre-licensing visit. Applicant stated that the adults living in the home are themselves, their spouse, and mother in law with three children ages 6, 15 and 14. The applicant provided proof of control of property. Applicant is unsure if they will get liability insurance and she understood that if she decides not to carry liability insurance in the future, she will have parent/authorized representative sign the Affidavit Regarding Liability Insurance for FCCH form (LIC 282). A review of staff records on 07/07/2023 indicates that all staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. The entire home was inspected for health and safety hazards. The home has a working smoke detector, a working carbon monoxide detector, a working telephone and a 1A10BC fire extinguishers, LPA informed applicant they will need to purchase a bigger fire extinguisher. Certifications for CPR and First Aid expire 12/17/2024. Preventative Health Practices training was complete on 12/10/22. Applicant has completed Mandated Reporter training on 09/28/2022. Immunizations for Measles, Pertussis, and influenza are on file for applicant. Continues on report dated 07/07/2023 ______________________________ pg. 1/3 Continuation of report dated 07/07/2023 ____________________________ pg. 2/3 LPA observed a safe with fire arms at the home, ammunition is stored separately. LPA observed two dogs and was informed they are vaccinated. LPA observed a fireplace that needs to be gated, no stairs and no bodies of water. Off limit areas: three bedrooms and backyard, left side yard will be used for outdoor time. The days and hours of operation will be Monday through Saturday from 6:00 am to 6:00 pm. There were age appropriate toys in the childcare areas . Bathroom was clean. Cleaning products, hazardous and sharp objects were inaccessible to children and stored in the laundry closet and top kitchen cabinet. Discipline policy was discussed with applicant and she understood that the children's personal rights should not be violated and corporal punishment is not allowed. Applicant stated that her discipline method will be redirecting and time out. Applicant stated they are unsure if they will transport children; they understood the safety seat belt/car seat requirements. LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant. A Family Child Care Home packet was reviewed with applicant. Department's website: www.ccld.ca.gov to obtain forms and regulations CCR, Title 22 was provided. Applicant was advised on the regulations that smoking, baby walkers, bouncers, excersaucers and other similar items are not allowed. The following was discussed, isolation of sick children; supervision of children; capacity options; requirements for reporting suspected child abuse; unusual incident/injuries; fire drills practiced at least twice a year and documented. LPA also discussed the requirements of AB 633 to applicant and provided applicant the fact sheet and a copy of Acknowledgement of receipt of Licensing Reports (LIC 9224). Zero Tolerance deficiencies with $500 civil penalty was also explained to applicant. LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. LPA also informed[applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Continuation of report dated 07/07/2023 ____________________________ pg. 3/3 Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. On this date, 07/07/2023, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Exit interview conducted and report was reviewed with the applicant, Blanca Sabalsa. LPA advised applicant that licensure for a small Family Child Care Home license is approved pending the following: - Minimum size of 2A10BC Fire extingushier - Fire place needs to be made inaccessible - Management's approval
