What the inspector wrote

On 04/21/2026, Licensing Program Analysts (LPA) Jennifer "Jen" Beehler conducted an announced pre-licensing inspection. Upon arrival, LPA was greeted by the applicant Maria Fabian Elizalde and provided access to the home. This inspection is in response to the Applicant's relocation application request. The applicant was the only person present today. The applicant stated her partner and herself are the only adults living in the home. There is a board to post required postings, such as notification of parent's rights. The hours of operation are Monday through Friday 5AM to 6PM. Applicant will be using her cell phone to communicate with parents. Applicant understands that she must have parents sign the Affidavit Regarding Liability Insurance as she does not carry liability insurance at this time. LPA toured the inside and outside of the home with Applicant. The off-limit areas inside are the garage and the entire upstairs. A gate was in place at the base of the stairs making it inaccessible from children in care. Disinfectant, cleaning supplies, and other items that could pose a risk to children were stored in the garage. There are games, activities and equipment for children. There were no babies walkers observed during today's inspection. There is a working smoke detector, and carbon monoxide detector. LPA observed a fully charged 3A40BC fire extinguisher serviced 04/01/2026 which is compliant with state fire marshal regulations. LPA reminded applicant that a fire extinguisher must be serviced each year or purchased new. Applicant stated that there are no weapons, such as firearms, stored on the premise. The hall bathroom on the main level will be used for children in care. LPA observed it to be clean and in working condition. Continued on Page 2 Applicant plans on using the foyer near the bathroom and front door to isolate sick children. The backyard can be used for outdoor play and is fenced. The off-limit areas outside are the front yard and one of the side yards. LPA observed building overhang to provide shade. There was astroturf and seating for children. There were no bodies of water observed during today's inspection. Applicant states that she intends on caring for children that are 0 years to 14 years old. Currently she has no infants in care only school age children. LPA discussed the safe sleep regulations with Applicant, Maria Fabian Elizalde and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant stated she does not provide medical services or administer medication. Incidental Medical Services (IMS) policy was discussed. For IMS information , see PIN 22-02-CCP . When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Applicant and partner are currently the only adults 18 and over living in the home. Continued on Page 3 Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. All adults in the home have criminal record clearance and are associated to the facility roster on Guardian. All adults also have a negative TB test along with updated influenza vaccinations on file with the Department. LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. On 04/21/2026, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Applicant understands that she cannot be absent more than 20 percent of her day care hours each day. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Applicant stated she has signed up for the information notices and does receive them. Proof of an updated influenza vaccination for Applicant's partner was collected during today's inspection Exit interview conducted and report was reviewed with the applicant, Maria Fabian Elizalde. Applicant was informed that upon review and approval from a Licensing Program Manager a license to relocate to this address would be granted.
