What the inspector wrote

On 4/23/26 Licensing Program Analyst made an unannounced case management visit to the facility and met with Tammy Harbison, Director (D1) for the purpose on following up on a request for change of capacity. D1 stated that the facility is not longer interested in changing the capacity of their toddler program and wishes to withdraw their application. However, during the visit LPA discovered the licensing fee's for the facility's sister location were unpaid. D1 stated that she is responsible for paying the fees at both locations. LPA observed D1 pay the licensing fees for the sister location (FN 283006616). The licensing fee for FN 283010861 is current. No Title 22 deficiencies were cited during this inspection. Appeal rights were provided. This report was reviewed and discussed with the D1. A Notice of Site Visit shall be posted for 30 days from today’s visit. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

    Based on LPA record review, the licensee did not comply with the section cited above in 3 out of 3 records reviewed in which the facility is not maintaining 15 min sleep log checks. Staff are present in the room when children sleep but are not recording the checks which poses/posed a potential health, safety or personal rights risk to persons in care.
