At 10:00am on 10/14/2025, Licensing Program Analyst (LPA) Matthew Gallo arrived at the facility to open a complaint investigation. Upon arrival, LPA observed a census of 30 preschool children supervised by 3 staff. Director Donyale Miller arrived at approximately 11:00am. The two classrooms for the facility are located in rooms M1 and M2. At 10:32am, LPA entered classroom M1 and observed 15 preschool children with one staff member (S1). Upon being asked if they have an assistant today, S1 stated that their assistant (S2) had left for the bathroom about 3 minutes prior to LPA arriving. When LPA left the classroom at 10:55am, S2 had not returned and no additional staff had joined S1 in ratio. Therefore, the facility was out of ratio during that time. A Title 22 deficiency is cited on the following LIC809-D Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Exit interview conducted and report was reviewed with the facility representative, Donyale Miller. A notice of site visit was given and must remain posted for 30 days.
