At 1:10pm on 11/7/2025, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Scott Mikal-Heine to conduct a plan of correction visit related to citations issued on 10/14/2025. Director Donyale Miller joined the visit for the exit interview. During today's visit, LPA observed a census of 21 preschool children supervised by 4 staff. The licensee was previously cited a Type A deficiency on 10/14/2025 for being out of teacher-student ratio. The plan of correction stated that the facility representative would conduct training in ratio and would provide an agenda signed by participants, and that LPA will return to ensure compliance. During today's visit, LPA observed 21 preschool children split between two classrooms. In classroom M1, 13 napping children were supervised by 2 qualified staff. In classroom M2, 8 napping children were supervised by 1 qualified staff. The plan of correction has been fulfilled and is therefore cleared. Exit interview conducted and report was reviewed with the facility representative, Donyale Miller and Scott Mikal-Heine. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights. |
