Inspector’s narrative

What the inspector wrote

At 11:45am on 10/21/2025, Licensing Program Analysts (LPAs) Matthew Gallo and Lea Habtom met with licensee Jeannette Stillwell for the purpose of opening a complaint investigation. During today's visit, LPAs observed a census 12 children consisting of 6 preschool children, 3 school aged children, and 3 infants. Licensee's adult assistant was also present for the inspection. While taking census upon arrival, LPAs were made aware by licensee that a child was sleeping in a stroller beside the licensee. LPA Gallo walked around and observed that the child was strapped in and asleep. At the time, licensee stated the child had been asleep for a few minutes. Approximately 10 minutes later, LPA Habtom noticed the child in the stroller to which the licensee stated the child was still asleep. A Title 22 deficiency is cited on the subsequent page of this report. Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Exit interview conducted and report was reviewed with the licensee, Jeannette Stillwell. A notice of site visit was given and must remain posted for 30 days.

This poses an immediate health, safety, or personal rights risk to persons in care.
