What the inspector wrote

At 12:35pm on 12/19/2025, Licensing Program Analyst (LPA) Matthew Gallo arrived at the facility to close a complaint investigation. Upon arrival, LPA observed a census of 8 children consisting of 5 preschool children and 3 infants. LPA was initially met by licensee's assistant, and licensee arrived shortly afterward. During the course of the complaint investigation, LPA determined that an additional deficiency unrelated to the complaint occurred. Through interviews with relevant individuals, LPA learned that a child had sustained an injury at the facility that required attention by a medical professional. The injury was not reported to licensing as is required by Title 22 regulation. A Type B Title 22 deficiency is cited on the following 809-D Exit interview conducted a report was reviewed with the licensee, Jeannette Stillwell. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
