On 4/23/2026, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Sarah Aguilar to deliver findings to a complaint investigation into the above allegation. Today's census included 4 infants in care. Throughout the course of the investigation, LPA conducted observations and interviews concerning the allegation that facility did not discard spoiled food products upon expiration date. During an initial facility visit on 3/5/2026, LPA observed a half-gallon carton of store-bought milk in the fridge labeled with an enrolled child's name and featuring a printed expiration date of 2/26/2026. The carton was discarded during LPA's visit. The preponderance of evidence standard has been met; therefore, the allegation is SUBSTANTIATED. A Title 22 Deficiency is cited on the following 9099-D. Exit interview conducted and report was reviewed with the facility representative Sarah Aguilar. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
