At 10:00am on 11/21/2025, Licensing Program Analyst (LPA) Matthew Gallo met with licensee Jaaziel Heckathorn for the purpose of an unannounced annual inspection. Upon arrival, LPA observed a total census of 8 children consisting of 4 infants, 3 preschool children, and 1 school aged child. Licensee's husband and three minor children over the age of 10 were additionally present during the inspection. Licensee guided LPA on a tour of the facility, and a health and safety inspection was conducted in all areas accessible to children. Prior to visit, off-limits areas included master bedroom/bath, laundry room, and bedrooms 2&3 (the two bedrooms to the left of the living), and bedroom 4 (next to the master bedroom). During visit, licensee stated they wished to designate additional areas of the home and property as off limits. Moving forward, off limit areas will include: all bedrooms and the backyard property beyond fenced outdoor play yard. Licensee acknowledges that children must never enter these areas. LPA observed the required postings, a working phone, 2A10BC fire extinguisher, and functioning smoke and carbon monoxide detectors. Toxic and hazardous items are inaccessible to children. Per Licensee, there are no weapons in the home. There are no fireplaces or stairs in the home. Outdoor play space is fenced. A hot tub pool is located directly behind the home. Upon inspection, LPA observed that the pool did not have a cover that could lock while not in use and was not enclosed by a fence or gate that met all the requirements of Title 22 regulations. Report continues on LIC809-C Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA reviewed 8 children\342\200\231s files and observed them to be complete. A current roster is being maintained and fire and disaster drills are documented. Current CPR and First Aid certification was verified and expires 4/2026, and AB 1207 Mandated Reporter Training was verified for the Licensee and expires 11/2027. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. A Title 22 deficiency is cited on the subsequent pages of this report. LPA Matthew Gallo informed licensee Jaaziel Heckathorn that this report dated 11/21/2025 documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care . Report continues on 809-C Also, LPA Gallo informed the licensee to provide a copy of this licensing report dated 11/21/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A civil penalty of $500 is assessed on the following LIC421IM due the presence of accessible bodies of water on the property. Exit interview conducted and report was reviewed with the licensee , Jaaziel Heckathorn During the exit interview, the licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
