Inspector’s narrative

What the inspector wrote

At 9:35am on 11/25/2025, Licensing Program Analyst (LPA) Matthew Gallo met with licensee Jaaziel Heckathorn to conduct a plan of correction visit related to citations issued on 11/21/2025. During today's visit, LPA observed a census of 10 children consisting of 2 infants, 7 preschool children, and 1 school aged child. An adult assistant and two of licensee's children under the age of 18 were also present during the visit. The licensee was previously cited a Type A deficiency on 11/21/2025 due to a hot tub behind the home that was not made inaccessible by a locked cover or fencing that met licensing requirements. The plan of correction stated that the licensee would install latches with locking mechanisms by the POC due date. During today's visit, LPA observed that the latches had been installed and were key-locked. The plan of correction has been fulfilled; therefore, the deficiency is cleared. Exit interview conducted and report was reviewed with the licensee, Jaaziel Heckathorn. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
