What the inspector wrote

At 10:45am on 10/1/2026, Licensing Program Analyst Matthew Gallo met with licensee Rachaiel McKnight to conduct a case management inspection related to an unusual incident report received on 9/26/2025. Upon arrival, LPA observed a census of 9 children consisting of 5 preschool children and 4 infants. On 9/26/2025, LPA received a self-reported unusual incident report detailing an incident in which a now-dismissed assistant yelled at a child in care. Staff interviews conducted during today's visit provided supporting evidence that the dismissed assistant yelled at licensee's child, who is under the age of 10 and who was present during day care hours, as a means of correcting their behavior, and that the tone and volume of the yell abridged the personal rights of the child. The licensee dealt with the issue the following day by suspending the assistant and notifying parents of the incident. The assistant is not allowed to return to the facility. A Type A deficiency Title 22 is cited on the following LIC809-D. Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Exit interview conducted and report was reviewed with the licensee, Rachaiel McKnight. A notice of site visit was given and must remain posted for 30 days.
