At 9:20 am on 6/28/2024, Licensing Program Analyst (LPA) Matthew Gallo met with applicant Mikayla Sharp for the purpose of a change of location prelicensing inspection. Applicant has requested a Small Family Childcare Home License to serve up to 8 children, and was previously licensed at facility # 293624528. The facility is a two story, 3 bedroom 3 bathroom home. The downstairs consists of a living room, dining room, kitchen, bathroom, master bedroom and master bathroom, and garage. The upstairs consists of a bathroom and two remaining bedrooms. The backyard is fenced . Off limit areas include: garage. Applicant understands that children must never enter these areas. Applicant also understands that any unfenced areas of the property require 100% supervision. Applicant intends to operate 7:30am-5:00pm, Monday-Friday, year-round. Applicant understands they must be present at the facility for 80% of operating hours per day. Applicant understands that their own family members under 10 years old count toward the ratio and capacity of daycare children. All individuals subject to criminal background review have obtained a criminal record clearance. Applicant was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. All toxic and hazardous items are inaccessible to children and applicant understands that any cabinet containing such items and is reachable by children must be made inaccessible. LPA observed a 2A10BC fire extinguisher along with functioning smoke and carbon monoxide detectors. Applicant stated that there are no firearms in the facility, and LPA observed no bodies of water on the property. Stairway is fenced. Applicant provided proof of control of property in the form of a lease agreement. Applicant understands that until a liability insurance coverage in the amount of $300,000 is provided, the affidavit LIC282 form will be used. Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). Applicant\342\200\231s EMSA certified CPR and First Aid Certificate expires 2/2026. AB1207 Mandated Reporter Training certificate was verified and expires 1/2026. Applicant understands both these trainings must be renewed every two years. LPA reviewed Safe Sleep regulations and provided the Licensing Safe Sleep webpage at h ttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. This facility evaluation report was reviewed and discussed with the applicant, Mikayla Sharp. LPA discussed supervision, personal rights, criminal record clearances, ratios and capacity, required postings, and maintaining buildings and grounds. Applicant was encouraged to visit the Department's website at WWW.CCLD.CA.GOV for information regarding childcare updates, forms, regulations and legislation pertaining to family childcare homes. On 6/11/2024, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. As of today, June 28, 2024, this facility is approved for a Small Family Child Care Home license for a capacity of 6 children with no more than 3 infants, or 4 infants only; or up to 8 children with no more than 2 infants, with 1 child in Transitional Kindergarten or above and 1 child at least age 6. Infants are children under the age of 2.
