Inspector’s narrative

What the inspector wrote

At 10:20am on 4/29/2026, Licensing Program Analyst (LPA) Matthew Gallo arrived at the facility to conduct a case management visit related to a self-reported unusual incident. During today's visit, LPA observed a census of 3 infants supervised by 2 staff and 12 preschool children supervised by 3 staff. Facility representative Brittany Borden arrived shortly after LPA's arrival. On 2/5/2026, the facility self-reported an unusual incident that had occurred at the facility on 2/3/2026. According to the report, a staff member (S1) of the preschool component admitted to striking a child with their hand across the child's face after the child had kicked them. During today's visit, LPA interviewed staff members and received corroborating accounts of S1's admission. LPA determined that, upon admitting to facility management that they had struck the child, S1's employment was immediately terminated on the day of the incident. Staff interviews described the facility as regularly discussing discipline policy during staff meetings and in-service trainings prior to the incident, including the topic of de-escalation and the importance of personal rights. A Title 22 deficiency is cited on the following LIC809-D. LPA Gallo informed facility representative Brittany Borden that this report dated 4/29/2026 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Gallo informed the facility representative to provide a copy of this licensing report dated 4/29/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the facility representative, Brittany Borden. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
