During the 3/20/2025 visit, LPA also observed the barricade to the wood burning stove, which is located in a recessed wall section. One end of the barricade was anchored to a wall, while the other end was positioned in front of another wall, impeding a child from walking straight into the recessed area to the wood burning stove. Interview with staff did not produce evidence that children in care have tampered with the barricade or bypassed it to access the wood burning stove. Upon LPA's visit on 6/6/2025, LPA observed the barricade to be fully anchored to the wall on both sides. The preponderance of evidence standard has not been met; therefore, the allegation is UNSUBSTANTIATED. Exit interview conducted and report was reviewed with the facility representative, Amy Wright. A notice of site visit was given and must remain posted for 30 days. LPA provided facility representative with appeal rights.
