What the inspector wrote

At 11:20am on 11/19/2025, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Sarah Kilgour to open a complaint investigation. Upon arrival, LPA observed 7 toddlers and 2 preschool children supervised by 1 staff and 5 infants supervised by 2 staff. During today's visit, LPA observed a deficiency unrelated to the allegations of complaint. Upon arrival, LPA observed a single combined class of 9 children in the outdoor play area. Through record review, LPA determined that the youngest child, Child 1 (C1), was 23 months old, and the oldest two children, Child 2 (C2) and Child 3 (C3), were both over 46 months old. The class was identified by staff as being the toddler class, and LPA observed that they used the toddler classroom. The terms of the license dictate that children in the toddler classroom must be no older than 36 months. A Title 22 deficiency is cited on the following page of this report. LPA Matthew Gallo informed facility representative Sarah Kilgour that this report dated 11/19/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care. Also, LPA Gallo informed the facility representative to provide a copy of this licensing report dated 11/19/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the facility representative, Sarah Kilgour. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
