Inspector’s narrative

What the inspector wrote

At 12:40pm on 12/3/2025, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Sarah Kilgour for a plan of correction visit related to a citation issued on 11/19/2025. During the visit, LPA observed a census of 17 children, consisting of 9 preschool children in the preschool component supervised by 2 staff, and 8 infants in the infant component supervised by 2 staff. The licensee was previously cited a Type A deficiency on 11/19/2025 due to not adhering to the facility license's terms and conditions for the age of children in the toddler component, in the case of a 46 month year old co-mingling in the toddler component. The plan of correction stated that the licensee would call a substitute teacher to provide care in the preschool classroom, and that LPA would return to ensure compliance. During today's visit, LPA observed 9 preschool children supervised by 2 staff, with no toddler children in care. The two staff were both qualified teachers, capable of supervising separate preschool and toddler classrooms. The plan of correction is fulfilled; therefore the deficiency is cleared. No further deficiencies were observed during the visit Exit interview conducted and report was reviewed with the facility representative, Sarah Kilgour. A notice of site visit was given and must remain posted for 30 days. LPA provided appeal rights.
