What the inspector wrote

At approximately 9:25am on 4/8/2025, Licensing Program Analyst (LPA) Matthew Gallo met with licensee Lenee Durand for the purpose of a change of location prelicensing inspection. Licensee has requested a Large Family Childcare Home License to serve up to 14 children, and was previously licensed at facility # 293625848. No children were present during the inspection. The facility is a one story, 2 bedroom 2 bathroom home consisting of a Kitchen, Living Room, Dining Room, Office, Master Bedroom/Bath, Bathroom, Spare Bedroom, and Garage. The backyard is fenced and features a deck. Off limits areas will include: Master Bedroom/Bath, Spare Bedroom, Office, and Garage. Licensee acknowledges that children must never enter these areas. Licensee also understands that any unfenced areas of the property require 100% supervision when children are present. Applicant intends to operate 8:00am-5:00pm, Monday-Friday, year-round. Licensee understands they must be present at the facility for 80% of operating hours per day. Applicant understands that their own family members under 10 years old count toward the ratio and capacity of daycare children. All individuals subject to criminal background review have obtained a criminal record clearance. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Report continues on 809-C LPA and licensee toured the entire home and property. All toxic and hazardous items are inaccessible to children and applicant understands that any cabinet containing such items and is reachable by children must be made inaccessible. Licensee acknowledges that any poisons must be made inaccessible by key or combination lock. LPA observed a 2A10BC fire extinguisher along with functioning smoke and carbon monoxide detectors. Licensee stated that there are no firearms in the facility, and LPA did not bodies of water on the property. There is a gas stove heater that does not have the gas connected and is nonfunctional. Licensee acknowledges that if the gas is reconnected and the stove returns to being functional, a barricade is required to keep it inaccessible to children. There are stairs leading from the deck down to the backyard play area that are properly fenced. Applicant provided proof of control of property in the form of a lease agreement. Applicant understands that until a liability insurance coverage in the amount of $300,000 is provided, the affidavit LIC282 form will be used. Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). Applicant\342\200\231s EMSA certified CPR and First Aid Certificate expires 2/2027. AB1207 Mandated Reporter Training certificate was verified and expires 4/2025. Applicant understands both these certificates must be renewed every two years. LPA reviewed Safe Sleep regulations and provided the Licensing Safe Sleep webpage at h ttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. Report continues on 809-C Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. This facility evaluation report was reviewed and discussed with the licensee, Lenee Durand. LPA discussed supervision, personal rights, criminal record clearances, ratios and capacity, required postings, and maintaining buildings and grounds. Applicant was encouraged to visit the Department's website at WWW.CCLD.CA.GOV for information regarding childcare updates, forms, regulations and legislation pertaining to family childcare homes. On 4/8/2025, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Effective today, 4/8/2025, the facility is approved for a Large Family Childcare Home license to serve a maximum capacity of 12 children (when there is an assistant present) with no more than four infants, or a maximum capacity of of 14 children with no more than three infants, and one child in kindergarten or elementary school and one child at least age six. If an assistant is not present, the facility must adhere to ratio regulations of a Small Family Childcare Home license. Applicant acknowledges that their license for facility # 293625848 is now void, and child care can no longer be provided at that location.
