What the inspector wrote

Page 1 of 2 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of decreasing the capacity of preschool program by taking off one of the current classroom (Room # 2) off of the license. The facility has requested to decrease their capacity from 85 to 80 children in Rooms # 3, 103, 112. and 115. Room # 115 is for 12 school age children with a granted waiver. LPA met with director, Deanne Walkwitz who gave a tour of the school to LPA. Currently the facility is licensed for 85 children including 12 school age children. LPA observed there were 6 staff with 32 children and 11 school age children in Room # 115. The facility is on the premises of The Evangelical Free Church of Orange. Room 103, 112, and 115 are located in building on two adjacent properties with different addresses. There is a waiver on file for these two programs to operate under one license for each program category. All these rooms are under the address of 1400 E. Taft on the license. A review of the Facility Personnel Report Summary indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The facility has requested to provide care and supervision to 80 preschool children including 12 school age children, Monday to Friday from 6:30 am to 6:00 pm. The children are dropped off, signed in and out electronically at the entrance of the building to go to rooms. The facility has met all the posting requirements. Both rooms were measured. The indoor activity space measurement for the current requested capacity is as follows: Room # 3 is: 899.39 square feet. Room # 103 is: 695.84 square feet. Room # 112 is: 812.28 square feet Room # 115 is: 668.10 square feet (School age with granted waiver) Total indoor measurement is: 3075.61 divided by 35 square feet = 87.87 (88 children). Continued on page 2 Page 2 of 2 Total outdoor measurement is: 11955 square feet divided by 75 = 159.40 (159 children). There are adequate cushioning materials (wood chip) under climbing structure, swings, and slides on the plagyround. Total of toilets 9 X 15 = 135 children Total of sinks: 11 X 15 = 165 children The facility is equipped with carbon monoxide detector. All the classrooms have age appropriate toys and equipment. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the director, Deanne Walkwitz. Based on today\342\200\231s indoor and outdoor measurements, the sinks and toilets availability, and per fire department the request for decreasing capacity to 77 preschool children including 12 school age children with a granted waiver in rooms 3, 103, 112, and 115 (Room # 115 school age) will be final reviewed and discussed with management in the Regional Office and after receiving the following.
