On 10/27/2025, Licensing Program Analyst (LPA), Deschampe conducted an on-site inspection for the purpose of an Unannounced Annual Random inspection. LPA arrived at the facility at 10:43 AM and was allowed entry by the facility representative/assistant director, Valorie Mageean. Facility representative guided LPA on a tour of the facility and was joined by facility representative/director, Terri Martinez at 11:14 AM who completed the tour of the facility with LPA. During the tour of the facility, LPA took the following census: Lower level room had 9 children with 1 staff, lower level patio area had 9 children with 1 staff, and lower level playground had 7 children with 1 staff. The upper modular levels had 9 children with 1 staff in Pre-K Prep 2, Pre-K Prep 1 had 9 children with 1 staff, and Pre-K had 9 children with 1 staff. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7:00 AM - 6:00 PM, Monday through Friday. A review of the Facility Personnel Report Summary on this date, 10/27/2025 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Due to time constraints only the tour of the physical plant and record review was completed. No deficiencies cited during this visit. Exit interview conducted and report was reviewed with the facility representative/director (FR), Terri Martinez. Appeal Rights were discussed with facility representative. The facility representative was provided a copy of the appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. A notice of site visit was given and must remain posted for 30 consecutive days.
