What the inspector wrote

On 4/15/2026 , Licensing Program Analyst (LPA) K. Trinh conducted an unannounced case management- incident visit in response to a self-reported unusual incident report (LIC 624) received on 3/31/26. LPA met with Director, Corinne Preisler. A walk through of the facility was conducted, and census was taken. Total census was 18 preschool children, 5 parents, and 1 aide . A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 3/31/26 , a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported that on 3/23/26 or 3/25/26, Child 1 (C1) was hit on the head by Child 2 (C2) with a frying pan. During today's inspection, LPA Trinh interviewed two (2) staff, two (2) children, and two (2) parents. LPA also obtained pertinent documents . Due to additional information needed, the reported incident needs further investigation. Exit interview conducted and report was reviewed with Director, Corinne Preisler. A notice of site visit was given and must remain posted for 30 days. End of report.
