What Happened
A case management tele-inspection found a clearance association problem for one staff member. The staff person had a clearance, but the clearance was not associated to the facility as required.
Observations
- Staff member Taylor Faust had a clearance but was not associated to the facility.
- The director said a request to associate the staff member had been faxed earlier, but no proof of receipt was available.
