What Happened
Case management incident inspection following self-reported incident. Child #1 left in bathroom stall for 1-2 minutes without supervision when staff failed to accompany child to restroom. Type A citation issued for supervision violation. Facility implemented revised bathroom procedures for afternoon hours.
Observations
- Child #1 left unsupervised in bathroom stall for 1-2 minutes on 11/14/2024 at 3:45 PM
- Staff #2 heard child crying and banging from bathroom area
- Staff #2 found child standing at closed bathroom door
- Staff #2 walked child back to playground and notified Staff #3
- Staff #1-S3 confirmed parent was notified
- Facility changed bathroom procedures for afternoon (3:30-6:00 PM)
