Inspector’s narrative

What the inspector wrote

(Page 2 of Report) During investigation, LPA Tran interviewed 6 staff on 05/24/2023 and 5 children on 06/20/2023. During staff interviews, all interviewed staff denied that they have or have witnessed any staff at the facility, ever used foul language in front of the children nor they have witnessed the Director used foul language to refer to anyone. All interviewed staff denied that facility administrator ever instructed them not to report any incident but to make sure they inform the admin staff before making the report. During children interviews, all interviewed children denied that they ever heard any staff used foul language. Based on the interviews conducted with 6 staffs and 5 children, there is insufficient evidence to corroborate the allegation that Facility administrator does not following proper reporting requirements and facility administrator speaks inappropriately in the presence of day care children. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that Facility administrator does not following proper reporting requirements and facility administrator speaks inappropriately in the presence of day care children, did or did not occur, therefore the allegations that facility administrator does not following proper reporting requirements, and facility administrator speaks inappropriately in the presence of day care children, are UNSUBSTANTIATED. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Rebekah Bay. (End of Report)
