On 09/18/2025 at 8:30am, Licensing Program Analyst (LPA) Sarah Garcia conducted a case management- incident visit in response to a self-reported unusual incident report (LIC 624) received on 08/14/25 regarding allegedly lack of supervision leading to an inappropriate child on child interaction. LPA met with director, Aysegul Inceoglu. A walk through of the facility was conducted, and census was taken. Total census was 51 preschool children and 8 staff. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 08/14/2025, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported that on 08/07/25 during after school care between 3-5pm, Child 1 (C1) disclosed to Parent 1 (P1) that the C1 took off clothes with another child in the classroom and teachers in the classroom (Staff #1 and Staff #2) were not allowed to see. During today's inspection, LPA Garcia interviewed 3 staff and attempted to interview C1, and obtained C1's emergency information (LIC 701), and other relevant documentation. Due to additional information needed, the reported incident needs further investigation. Exit interview conducted and report was reviewed with Director, Aysegul Inceoglu. A notice of site visit was given and must remain posted for 30 days.
