Page 1 On 12/12/2023, Licensing Program Analysts (LPAs), Anna Chan and LPA Karen Navar conducted an onsite inspection for the purpose of a 3-year inspection Visit. LPA met with Director Jennifer Gates. LPAs and Director toured the facility inside and outside and the floor and yard plan were verified. Census was taken in individual classrooms. The overall census observed was 12 infants and 4 staff. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7a.m.- 6p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Age-appropriate materials such as books, puzzles, blocks are provided. Food is brought from home. Food prep areas were clean and sanitary. Food is properly stored. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by drinking water using Brita pitchers and sports bottle, cups, are also brought by children from home with the child\342\200\231s name on it. Infants sleep in cribs or cots, and bedding is stored individually. Blankets and sheets are brought home on the last day a child attends school and brought back on child\342\200\231s first day back in school. The facility has conducted an emergency drill in November 8, 2023. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirements. The California Child Passenger Safety Law was posted by the entrance of the facility. Continue Page 2 Page 2 The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height.The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material is grass and appeared to be enough to absorbs falls. There are pretend houses and blocks around the playground. Drinking water in the outdoor activity space is provided by Brita water pitcher and sports bottles with the child\342\200\231s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair. 5 staff files were reviewed for staff present during the facility inspection on this date. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health, and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a child care center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis and measles for the facility were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporter training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 04/19/25. 5 Children\342\200\231s records were reviewed for a copy of the emergency information card that contains all the information specified by regulation (LIC 700), Parents rights (LIC 995A), consent for emergency medical treatment (LIC 627), Infant Sleeping Plan (LIC 9227), 15-minute infant sleep log, and immunization records were found to be in compliance. Procare app is used to sign in and out. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 , requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. \342\200\242 CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE Continue Page 3 Page 3 LPA referred Jennifer Gates to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the facility representative. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Based on observation and interview a Type B violation was observed and being cited in accordance with Health and Safety Code Section 1597.16(a)(1) Lead Testing. Facility not having lead test. An Inspection and exit interview were completed with Director Jennifer Gates. The report was reviewed and discussed. Appeal Rights and deficiencies were discussed. The facility representative was provided with a copy of their appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The facility representative was informed that the \342\200\234Notice of Site Visit\342\200\235 must be posted for 30 consecutive days. The \342\200\234Notice of Site Visit\342\200\235 must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00. End of Report
