On 10/23/24, Licensing Program Analysts (LPAs), Anna Chan and Alma Castro conducted an unannounced onsite inspection for the purpose of an Annual Random Visit. LPAs met with Preschool Teacher, Katie Fields at 8:50am and toured the facility inside and outside and the floor and yard plan were verified. Census was taken. The overall census observed was 17 children and 3 staff. Children were playing outside when LPAs arrived. Center director, Heidi Shikuma arrived to meet with LPAs at approximately 10:45am and continued with the visit. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7:00am-6:00pm., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. The facility does provide food to children. Facility provides snacks and lunch. Food prep areas were clean and sanitary. Water is provided by purified water pitchers and children bring sports bottles with children\342\200\231s name on it. Children nap on mats, and bedding is stored individually. Sheets and blankets are washed weekly by the facility. The facility conducted an emergency drill on October 08 th , 2024 at 11:00am. The facility has a working carbon monoxide detector and fire extinguisher. Facility met all posting requirements. The California Child Passenger Safety Law was posted by the entrance of the facility. Page 1 of 3 The outdoor activity space was inspected for compliance. The preschool playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The playground has a cushioned by pour in place rubberized cushioning around the play area which appears to be enough to absorb falls. Drinking water in the outdoor activity space is provided by purified water pitcher and children bring sports bottles with the children\342\200\231s name on it. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility, and none was observed during today\342\200\231s visit. 3 Staff files were reviewed for staff present during the facility inspection on this date. Health screening and immunization as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1596.7995 (a)(1) a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for the facility were reviewed and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporter training, and to renew the training every two years. 3 of 3 staff were missing current mandated reporter training. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 08/2026. 5 Children's records were reviewed, and there was a separate, complete, and current record for each child. In the areas reviewed the children\342\200\231s files were found to be in full compliance. Sign in/out procedur e through a physical binder. Each child has their own tab (labeled by last name, first name). Binder was reviewed and within compliance. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 , requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was not completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP , but facility tested and passed on 02/03/2023. This facility provides Incidental Medical Services (IMS) LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. See violation on the attached LIC 809D. An Inspection and exit interview were completed with Heidi Shikuma. The report and deficiencies were reviewed and discussed. Appeal Rights and deficiencies were discussed. The director was provided a copy of their appeal rights (LIC 9058 ) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. The director was informed that the \342\200\234Notice of Site Visit\342\200\235 must be posted for 30 consecutive days.
