Page 1 of 3 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of increasing capacity from 62 preschool children to 80 preschool children by adding room # 6 to the license. LPA met with Director, Talin Wartan. Census was taken. Currently the facility is licensed for 62 preschool children in rooms # 1 and 2, 3, 4, and 5. The licensee has requested to provide care and supervision for 80 preschool children ages 2 to 5 years old, Monday through Friday, 8:00 a.m. to 5:30 p.m. Today the total census were 49 preschool children with 7 staff. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Director, Talin Wartan was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Continued on page 2 Page 2 of 3 The indoor activity space measurement for the current requested capacity is as follows: Room # 1: 390.4 square feet. Room # 2: 415.91 square feet Room # 3: 722.64 square feet Room # 4: 383.55 square feet Room # 5: 403.97 square feet Room # 6: 476.80 square feet Total indoor activity space is 2793.87 square feet divided by 35 = 79.82 (80 children). Classrooms 1, 2, 3, 5 have their own toilet and sinks. There is a waiver on file for classroom # 4 to share the restroom across the classroom with elementary children. This waiver needs to be updated by adding one more toilet and one more sink of this restroom for usage of children from room # 6. Age appropriate toilet seat and step stools have been placed in this shared restroom. Total sinks: 9 X 15 = 135 Children Total toilets: 6 X 15= 90 (Children) including 2 stalls to share in this restroom with elementary children. Outdoor space measurement is: 3375 square feet divided by 75 = 45 children. A waiver is on file to use staggered schedules for outside time on file. This waiver needs to be updated to add the schedule of room # 6 to it. The facility is equipped with carbon monoxide detector. The restrooms and playground were inspected. There were adequate cushioning materials under and around the climbing structure on the playground which is all fenced. . Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP . A Plan of Operation that includes IMS must be submitted to the Department. Based on today\342\200\231s measurements of indoor and outdoor spaces, the sinks and toilets availability, the request of the facility for increasing the capacity to 80 children will be final reviewed and discussed with management in the Regional Office. The facility will be notified if any additional information is required. The following are needed to be sent to our office 1- An updated waiver letter request for sharing the restroom with elementary children. 2- An updated waiver letter request to use the playground on a staggered scheduling. Schedule for children of new room # 6 shall be added. Please make sure you send the request for the waiver with a copy of LIC 999, LIC 500, and a picture of the playground and restroom with the signs posted for "Preschool". A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the director, Talin Wartan. An updated license will be issued for 80 preschool children ages 2 to 5 years old, Monday to Friday from 8:00 am to 5:30 pm in rooms # 1, 2, 3, 4, 5, and 6 after receiving above and final reviewing and approval from the management.
