What the inspector wrote

On 3/20/2024, LPA interviewed four (4) staff and on 5/9/24, LPA interviewed four (4) staff. 7 staff indicated they did not witness a staff handle a child in a rough manner. On 5/13/2024, LPA called parents six (6) parents and reached two (2) parents. None of the parents disclosed any information that could corroborate the allegation. Due to conflicting information and a lack of supporting evidence obtained throughout the course of the investigation, the allegation is determined to be unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Exit interview was conducted with Director Rebekah Bay. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The Director was provided with a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above. Page 2 of 2
