LPA Anna Chan and Olivia Meza interviewed staff at the facility. (1) Staff interviewed stated they follow diaper changing routine which is every 2 hours or as needed, and staff stated they log every diaper change in their Procare app. Staff stated they routinely check diapers for infants and toddlers throughout the day. (2) Staff redirect children during challenging behavior, and they have an open communication with children’s authorized representatives. Based on daily diaper log review, document shows diaper change times, and staff follows a diaper schedule and logs any diaper changes. Based on record review, the facility follows Parent Handbook, and reports incidents or ouch reports to parents and documents incidents. On 12/17/24, LPAs observed staff talk and communicate appropriately to children during indoor activities and in the play yard. LPAs observed staff would redirect a child to a different toy if a friend were already playing with the toy, or redirect to another activity. None of the parents interviewed disclosed any information that supported the allegations. Based on i nterviews conducted and records reviewed, the preponderance of evidence standard has not been met. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations were UNSUBSTANTIATED. An exit interview was conducted with Director, Rebekah Bay. The Notice of Site Visit was posted during the visit. The director was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The director was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First-level appeals should be sent to the regional manager to the address listed above.
