What the inspector wrote

On 5/22/2025, Licensing Program Analyst (LPA) Olivia Meza, conducted a case management deficiency inspection. Upon arrival, LPA met with Regina Keith. There were a total of 57 children in care and Eight (8) Staff members. Based on LPA's observations, camera footage was reviewed from the date of May 16th. The camera footage showed two children not being supervised in the hallway. One Type A deficiency was cited per the California Code of Regulations, Title 22, Division 12 Section101439.1(f); see attached LIC809D. LPA Meza informed Director, Regina Keith that this report dated 05/22/25 documents 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. LPA Meza also informed the Director, Regina Keith, to provide a copy of this licensing report dated 05/22/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview was conducted with Licensee, Regina Keith. The Notice of Site Visit was posted for no less than 30 consecutive days. Appeal Rights were provided. **This is an amended version of the report**
