What the inspector wrote

Licensing Program Analyst (LPA), Alanna Gontarek conducted an onsite visit for the purpose of a 1 year required inspection. LPA and Site Supervisor, Alberto Oseguera toured the facility inside and outside and the floor and yard plan (LIC 999) were verified. Census was taken in individual classrooms. The overall census observed was 0 school age children and 1 school-age staff upon entry . During the inspection, 8 school age children signed into attendance. It was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7a.m.- 6p.m., Monday through Friday. Facility Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. LPA observed cleaning compounds to be stored in latched cabinets in both school age rooms. Poisons/Hazardous Items are not kept on the premises. Food is prepared on site; breakfast, lunch and snacks are provided. Food prep areas were clean and sanitary. Food is properly stored. Menus were posted where they could be reviewed by parents. Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by cups and pitcher . The children's bathrooms are clean and sanitary. Per Site Supervisor, there are no children that nap. The facility has conducted an emergency drill within the past six months. Last earthquake drill was conducted on 2/9/22, and last fire drill was conducted on 3/15/22. The facility has a working carbon monoxide detector and fire extinguisher. The fire extinguisher was last serviced on 7/7/2021. Facility met all posting requirements. The California Child Passenger Safety Law, License, Parent Rights poster, and Emergency Disaster plan (LIC 610a) was posted by the entrance of the facility. The outdoor activity space was inspected for compliance. The playground was enclosed by a fence at least four feet in height. The surface of the outdoor activity space was well maintained and free of hazards. The cushioning material Page 1 Page 2 is a soft foam cushioning, and is positioned around the climbing equipment, swings, slides and other similar equipment appeared to be enough to absorb falls. Drinking water in the outdoor activity space is provided by drinking fountain and cups and pitcher. The outdoor equipment and toys were in good repair and free of sharp edges. There are no bodies of water present at the facility. The facility grounds were safe, sanitary and in good repair. Staff files were reviewed for staff present during the facility inspection on this date, 2 out of 2 staff files were reviewed. Health screening and immunizations as required were reviewed. Beginning September 1, 2016, Health and Safety (H&S) 1596.7995 states, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against influenza, pertussis, measles for Site Director and staff were reviewed and within compliance. The Facility Representative does have proof of immunization against pertussis, measles, and influenza . Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed child care providers, administrators, or employees of a licensed child day care facility to complete the mandated reporter training , and to renew the training every two years. Staff reviewed have a current Mandated Reporter Training certificate on file. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 5/20/2023 . Records were reviewed using KIT #1. Children's records were reviewed, and there was a separate, complete and current record for each child. A random sample of children's files were reviewed for documentation of the child’s name, address, and telephone number of the child’s authorized representative and of relatives or others that can assume responsibility for the child if the authorized representative cannot be reached when necessary (Emergency Form) and a medical assessment . In the areas reviewed the children’s files were found to be in full compliance. Sign in/out procedure was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA observed medications to be stored in a code-locked box, and stored in a latched upper cabinet in both school age rooms. Child Care Advocate: at childcareadvocatesprogram@dss.ca.gov or at www.ccld.ca.gov Page 3 LPA provided Guardian Information and website info: https://www.cdss.ca.gov/inforesources/cdss-programs/community-care-licensing/caregiver-background-check/guardian LPA advised Facility Representative to register for Guardian to review facility rosters and to associate individuals. Information on the additional nutrition training, immunization requirements for children, and Health Schools Act: (http://www.cdpr.ca.gov/docs/pestmgt/schoolipm.htm ) were provided. The Facility Representative was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org LPA observed The California Department of Social Services Lead Information Brochure posted on Parent Board. LPA explained and provided External Water Sampler Self-Certification Form (LIC 9275) and the Child Care Center Sampling Checklist Form (LIC 9276), and Facility Representative was advised to have water tested for Lead by January of 2023. Facility Director does not have lead training Certificate on file. LPA advised Facility Representative to complete the Lead Training course. In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit. End of Report
