What the inspector wrote

RP stated the diaper rash was so bad that C1 was crying and could not walk properly. RP stated these incidents were discussed with the facility director. RP stated that C1 is often “starving” and “water bottle is bone dry” at pickup. RP spoke with the facility administrator about making sure C1 is “fed, hydrated, and changed.” RP also stated that the facility is not kept clean due to no longer having a “cleaning crew.” Lastly RP stated that in the mornings RP has observed, two teachers “talking to each other in the doorway of a class instead of watching the kids” on the playground. During the course of investigation, LPA interviewed 6 staff members, 3 parents and 5 children. During the staff interviews, Staff 1 (S1), disclosed that staff change a child’s diaper every 2 hours and in between if needed. If a child has a rash and parents request, staff will change the child every hour depending on the rash. When a child has a diaper rash, if the parent does not say anything, and the teacher notices and will inform the parent. Parents must fill out a form to allow staff to apply diaper rash cream. If parents don't fill out the form, staff will inform them about the rash and encourage them to fill out the form. If they did fill it out, then the cream cannot be expired. If the cream is expired, they would need to fill out a new form. Staff cannot apply ointment without the form filled out by parents. Diaper changes are documented on the app, “Sprout About”. As soon as they change them it’s updated on the app but for older children, teachers can update it at naptime or when things slowdown in the day. S1 stated if the child happens to be older, they get changed standing up so observing a rash is a bit harder because it’s harder to see. S1 disclosed that a child could potentially go home with a full diaper if that child gets picked up at the end of the 2 hour mark or if the child (an older child) denies the diaper change. 6 out of 6 staff disclosed that staff change diapers every 2 hours and as needed. Staff then document on the app. 6 out of 6 staff stated if a child has a diaper rash, they notify the parents and check if parents filled out the form that allows staff to apply diaper cream. If parents did not, then staff cannot apply cream. S1 stated that water provided to children by parents bringing sippy cups for each child. They are refilled every time a child asks it gets refilled and during mealtimes. There are water pitchers to refill the bottles, staff also check at naptime, refill and before going outside. If a child does not have their own water bottle, plastic cups are provided and there are extra sippy cups for toddlers. S1 stated that the snack/meals provided to children are the appropriate portion and they’ll get more if they want more and if more is needed, staff can request more from the kitchen. Page 2 of 4 If a child does not want to eat, there are other options and if staff know a child is picky, staff will ask parents if they can bring their own snacks, but it must be nut-free and nutritional. Parents are notified via app. 6 out 6 staff stated children bring their own water bottles and staff refill them throughout the day. Every class has a water pitcher to refill the bottles. If a child does not bring the water bottle, staff have plastic cups available. 6 out 6 staff stated children are served enough snack and more is provided if needed. If children want more food, they are given more. If a child does not eat, they are still served and encouraged. If a child still does not want to eat, staff will inform parents on the app and are encouraged to bring snacks from home. S1 disclosed in regards to supervision, staff conduct “name to face” counting and have a “name to face” document, there is a section for transitions and teachers must document that on the form. When staff are outside, staff are to spread out and not sit down, constantly walk around and add aides to help outside with supervision. S1 denied observing teachers talk to one another in between doorways and not adequately supervising the children, during transitions S1 and staff #2 (S2) walk around in case teachers need additional support. S1 hasn’t seen anyone doing that. S2 stated, that staff are encouraged to walk around, spread out and not sit when watching the children outside, not stand together and do name to face. 4 out of 6 staff denied or witnessing any staff “talking to each other in the doorway of a class instead of watching the kids”. S1 stated in regard to cleaning the facility, the facility currently does not have a cleaning crew. The staff members are cleaning the classrooms, such as, wiping down the table, sweep, mop and vacuum the floor, and take out their trash, they are given time at the end of the day to clean. S1 checks to make sure whatever doesn’t get done, S1 will do it and so will S2. S1 also stated that if time allows staff will take turns cleaning the bathrooms but it is mainly S1 and S2 that clean then. Sometimes, they will offer the aides additional hours and they will help as well. S2 stated at the end of the day the teachers clean their classrooms. Whatever teachers may not finish S2 and S1 will finish for them. Staff 3 (S3) stated, staff clean 3 times a week, deep cleaning, we mop, and vacuum. Disinfecting throughout the day, before and after meals and after activities. S3 has never seen the facility to be unsanitary for children. Staff #4 (S4) disclosed that staff clean the classrooms daily, staff maintain the facility sufficiently clean. Staff #5 (S5) stated staff clean 3 times a day for meals and after activities. Staff are given time to clean. Staff #6 (S6) disclosed that staff clean after every meal, wiping down tables, sweeping, cleaning up as they go. S6 thinks everybody tries to do their part. Page 3 of 4 LPA interviewed 5 children and none of them made any disclosure regarding the above allegations. 5 out 5 children stated if a child wants more snack staff give them more. In addition, they are always provided water and teachers refill their water them when necessary. LPA contacted 7 parent and was able to interview. 3 of 3 parents interviewed had no issues or concerns with the daycare. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegation is UNSUBSTANTIATED. Exit interview was conducted, and report was reviewed and discussed. Notice of Site Visit was posted during the visit. The facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100 per day. The facility was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights. Page 4 of 4. End of Report.
