Based on observation and record review, the licensee did not comply with the section cited above. LPA observed fire and disaster drill log was last documented on 8/12/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. At 8:03 am while touring room #305, LPA observed an unlocked cabinet with bleach, comet, multi-use wood cleaner, raid, and other items that could pose a danger to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Based on staffs' files reveiwing at 3:20 pm, the director did not comply with the section cited above in 4 out of 5 staffs' files. 4 staffs' CPR and 1st aid were expired. Staff # 1's certificate is still current but the staff was not present for a short period of time during inspection which poses a potential health, and safety risk to persons in care.

On or before March 30, 2018, a person who , on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reported training provided pursuant to paragraphs(2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every 2 years following the date on which he or she competed the initial mandated reporter training. This requirement was not met as evidenced by reviewing 9 staffs' files today resulting missing certificates for 6 staff. The licensee failed to meet this section of the regulations. This is a potential hazard to the health and safety of children in care.
