What the inspector wrote

On 7/11/2025 an unannounced case management inspection was conducted on this date by Licensing Program Analyst (LPA) Karen Navar in response to a self-reported incident dated 6/26/2025. During today’s inspection LPA met with Teacher 3 Kaitlyn, Coppage. Teacher 3 stated today is a field trip day and that the assistant director should be arriving at about 9:45AM. LPA then met with Assistant Director Cecilia, Espinoza and Sr. Program Director Isabel, Salgado. A review of adult records indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Department received an incident report on 06/26/2025 stating that on 06/25/25 at 10:55AM during playground time 1 child (C1) was playing in the grass area and ate a mushroom and parents were called. During the investigation LPA toured the facility and outside playground area with the Assistant Director and Sr. Program Director. 3 (three) staff interviews (S1-S3) stated that supervision is keeping eyes on children at all times, rotating positions, and children play areas are coned off. 3 (three) staff interviews stated that S4 was near the incident and parents were called immediately. LPA reviewed document titled Parent Communication form that recorded the incident and stated parent was talked to. Based on the information gathered from the interviews with staff and child records review/interivew. The facility was in compliance of the California Code of Regulations, Title 22, Division 12. Exit interview conducted and report was reviewed with the Sr. Program Director Isabel, Salgado. A notice of site visit was given and must remain posted for 30 days. Failure to post will result in civil penalties of $100.
