On 6/01/2026, Licensing Program Analyst (LPA), Madeline Do and Licensing Program Manager (LPM), M. Malane conducted a case management inspection for the purpose of inspecting playground B for use during new playground equipment installation. LPA Do met with Director Carrillo and was led on a tour of the facility. There were 41 children and five (5) staff present. LPA Do inspected and measured playground B which is not sufficient for the capacity for use during new playground equipment installation on playground A. Director stated they will submit a waiver request due to insufficient outdoor space and may submit a waiver request to share the back field with elementary students. LPA inspected and measured the field which is sufficient for the capacity. There is high climbing equipment that is not age-appropriate for preschool children which will need to be made inaccessible. Director stated they want to add a restroom off of playground B in the multi-purpose room for children’s use which was inspected. The restroom was sanitary and in working order. There was a sink in the room for children’s use. The following is needed prior to approval and construction: Updated LIC 309 Submission of waiver request(s) Proof of shade on playground B Proof of toys and materials for outdoor use Updated facility sketch with restroom and back field if being used Waiver approval from the Department Playground schedule for staggered use Exit interview was conducted and the report was reviewed with director Carrillo. A notice of site visit was given and must remain posted for 30 days.
