What Happened
Follow-up case management inspection for serious allergic reaction incident: child with known nut allergy given cashews by staff member, experienced severe allergic reaction, and staff failed to immediately administer EpiPen. Non-staff family member administered EpiPen. Type A citation issued.
Observations
- Staff member #1 gave child with documented nut allergy cashews during naptime and forgot about allergy
- Child experienced allergic reaction: scratching neck, eyes turned blue, vomiting occurred
- Staff did not immediately administer EpiPen despite authorization and training
- Parent's friend (non-staff member) administered EpiPen instead of facility staff
- Staff failed to seek immediate outside medical treatment
- Facility will post allergy list inside all classroom cabinet doors
- Facility will prohibit staff from bringing nuts or nut products into facility
- Facility will request additional staff training on EpiPen administration
