What the inspector wrote

Licensing Program Analyst (LPA) Patricia Duron conducted a Case Management Inspection to investigate an Unusual Incident Report (UIR) which was self-reported to our office on 10/20/2021. Upon entry to the facility, LPA Duron met with Center Manager, Marsha Linguist. LPA observed 15 school children in care with 4 staff members providing care and supervision. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The Self-reported incident received on 10/20/2021 stated (C1) left the Facility without informing Facility staff. Based on information that was obtained the Facility was unaware of C1\342\200\231s whereabouts for about 50 minutes. LPA Duron conducted this inspection to review facility\342\200\231s new procedures to ensure facility is providing care and supervision to children in care. LPA requested, the Facility Roster (LIC 9040), LPA observed Name to face log, sign in and out sheets,the facility's entrance/exit doors and an interview with Center Manager, Marsha Linquist was conducted. Center Manager stated every month staff members take a required Health and Safety Training. Manager stated during the school year, staff will contact front office and child\342\200\231s parents/authorized representatives if child does not arrive within 5 minutes after school has let out. Child's parents/authorized representatives will inform staff if child is enrolled in an after school program. Center Manager, Linquist stated, the elementary school provides a list to Catalyst Kids, when a child is enrolled in an after school program for example, homework help, A.C.E.S care an after school club for art or stem class, or sports. The Notice of Site Visit was posted. Center Management was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The Center Management was provided a copy of their Appeal Rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. Site Supervisor was informed all appeals must be in writing and received by the Licensing office within 15 business days. End of Report.
