On 07/11/2025, Licensing Program Analyst (LPA), Susan Deschampe conducted an onsite inspection for the purpose of an Unannounced Annual Random visit. LPA was greeted by the licensee, Irma Fisher and allowed entry to the Family Child Care Home (FCCH) at 9:17 AM. Census was taken in accessible areas of the facility. The overall census observed was 7 infants/children and 3 staff. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. Facility hours are 7:30 AM - 5:30 PM, Monday through Friday.
A review of the Facility Personnel Report Summary on this date indicates all facility residents, staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Currently, there are 3 adults, including the licensee living in the facility. There is one dog in the facility and the licensee states all vaccinations are up to date.
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
During today\342\200\231s inspection, LPA and licensee toured the inside and outside areas identified in the facility sketch as accessible to child care children. Off limits areas are made inaccessible by means of child safety

door knob covers and baby gates (outdoor). The licensee stated the primary area is the day care care room. There are working carbon monoxide detectors, smoke detectors, and 1 fire extinguisher in the home that meet the statutory and State Fire Marshall standards. Detergents, cleaning compounds, medicines, and other items which could pose a danger if readily available to children were stored inaccessible to children. Licensee stated there are no firearms and/or other dangerous weapons in the facility and none were observed during today's inspections. There is a fireplace in the living room made inaccessible to children in care with a tightly fitted piece of wood. The home has age appropriate toys for the ages served. LPA verified there is a working telephone service (cellular service). Licensee stated they use the backyard as an outdoor play area. The backyard play area was inspected and is free from hazards. There are no bodies of water on the premises.
The licensee does
have a current roster of children in care. LPA reviewed 7 out 7 children\342\200\231s records. Children\342\200\231s records for children present during LPA\342\200\231s inspection were reviewed for a copy of the emergency information card that contains all the information specified by regulation (LIC 700) and found to be in compliance. LPA observed and reviewed LIC 9227 Individual Infant Sleeping Plan in children\342\200\231s files. Infant Safe Sleep Logs were in compliance. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at:

, as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
The licensee and helper\342\200\231s Pediatric CPR/First Aid certification, expires 05/21/2026. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis and measles for licensee, spouse/helper, and daughter/helper were reviewed

and within compliance. Beginning March 31, 2018, H&S Code 1596.8662 requires all licensed providers and employees to complete mandated reporting training, and to renew the training every two years, expires 07/03/2026.

(b)(9) Evidence of a current tuberculosis (TB) clearance, not more than one year prior to or seven days after initial presence in the home, for any adult in the home during the time that children are under care.
This requirement was not evidenced by: 1 out of 3 staff files was missing proof of TB clearance.
Incidental Medical Services (IMS) are not provided at this facility. Incidental Medical Services (IMS) policy was discussed. The licensee understands they
must be present in the facility and must ensure children in care are always supervised. Children are not to be left alone in parked vehicles. When the licensee is temporarily absent from the facility, arrangements must be made for a qualified substitute adult to care and supervise children while absent. The substitute adult must have the required criminal record, child abuse index clearances, immunizations, Pediatric CPR/First Aid, and mandated reporter training.
Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Based on LPA observations, interviews, and record reviews the following violation was observed and is being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 3, Section

is being cited on the attached LIC 809D.

LPA discussed the Appeal Rights with the licensee. LPA provided a copy of the Appeal Rights (LIC
9058
). The licensee's signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
A notice of site visit was given to the licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
