LPA Duron conducted a Case Management inspection, on today's date 10/7/2025. The purpose of today’s case management inspection is to issue an amended report to update information dated 09/17/2025. LPA met with Director, Jordan Wolf. The overall census observed was 29 children with 4 staff members. A review of staff criminal clearance records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Exit interview conducted and report was reviewed with the Director, Jordan Wolf. A notice of site visit was given and must remain posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. End of Report.
