What Happened
Complaint investigation substantiated allegation that facility provided care to children without proof of immunization. Citation issued for Health & Safety Code 101220.1(a). Out-of-ratio allegation was unsubstantiated due to lack of evidence despite missing June records.
Observations
- two children (C1 attending since April 2024, C2 since September 2025) enrolled without immunization records
- no immunization exceptions documented for C1 and C2
- four of five staff interviewed unaware of immunization requirements upon enrollment
- facility director confirmed children attended without immunization proof due to delayed schedules
