What the inspector wrote

On 09/04/25, at 1:45pm, Licensing Program Analyst (LPA), Anna Chan conducted an unannounced onsite inspection for the purpose of measuring the Infant room B124. LPA met with Preschool Teacher Donelle Chavez. LPA informed Ms Chavez of the purpose of visit. LPA also met teacher Augustina Meza and was informed for the purpose of visit and LPA was led on a tour of the facility. Census was taken, the overall census observed were 18 infants and 7 staff. Infants were napping when LPA arrived. Office Manager Tiffany Cho Hoffman arrived and continued with the visit. The facility is currently licensed for 30 infants in rooms B115, B118 & B122. The facility is requesting to increase capacity to 36 infants and by adding Room B124 to their license. Indoor Space Activity Space (in sq ft): Room B124= 29.05 x 23.17 = 673.09 – 208.10 (encumbered space) divided by 35= 13 infants There was a separate napping area with 8 cribs and there were 3 high chairs in room B124. There were 2 sinks in the room; one for food preparation and one sink within arms reach of a changing table. Outdoor Activity Space was previously measured: Infant Playground: 29.2 x 32.8 = 688.38 sq. divided by 75= 9 infants Page 1 of 2 Infant playground will be utilized on a rotating schedule. Playground schedule was provided to LPA. The playground is fully enclosed by appropriate fencing. An adequate amount of cushioning material consisting of foam mat is in place under all play equipment. Adequate shade is provided by several canopies. During the visit, the infant room B124 is sufficient to accommodate the requested capacity of infants. A fire clearance was granted on 8/18/25, for 36 infant children: ages 6 weeks - 2 years for rooms B115, B118, B122, and B124 (all on the 1 st floor of the facility). An Inspection and exit interview were completed with Office Manager Tiffany Cho Hoffman. The report was reviewed and discussed. The file will be submitted to Licensing Program Manager for a final review. Notice of Site Visit posted and must be posted for 30 consecutive days.
