On 6/11/2025 Licensing Program Analyst (LPA), Olivia Meza, conducted an unannounced case management inspection to follow up on an incident that was self-reported to the Department. Upon arrival, LPA met with Director, Chantel Hamrick. LPA disclosed the purpose of the inspection and was led on a tour of the facility. There were seven (7) preschool teachers and 41 preschool children present. Hours of operation are Monday through Friday 6:30 a.m. to 6:00 p.m. On 05/21/24, facility reported to the Regional Office (RO) that a child was left on the playground. S(1) another teacher at the playground found the child. S(2) took the child to the classroom. S(3) Stated that a name to face procedure was done, but the child stayed outside as the class walked inside. On 06/11/25, LPA conducted a case management inspection and interviewed facility staff. LPA was provided with documentation pertinent to the investigation. The child was supervised by another classroom teacher while outside. The child was taken back to the correct classroom as soon as staff was notified. The school provided name to face protocol staff training and reported to licensing right away. Based on the information obtained and interviews conducted, the child was never left without supervision. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Chantel Hamrick and Assistant Director, Jacquline Sanchez.
