What the inspector wrote

On 10/23/2025, Licensing Program Analysts (LPAs) Navar conducted an on site required annual inspection. LPA and the Director Cheyenne Cazares toured the facility inside and outside, and the floor and yard plan were verified. Census was taken in individual classrooms. The overall census observed was 13 infants and 5 staff members. During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staff to child ratios. Facility hours are 7:00a.m.- 6p.m., Monday through Friday. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. During the inspection of the indoor activity space, items which could pose a danger to children (detergents, cleaning compounds, and medications) were observed to be stored out of the reach of children. Poisons/Hazardous Items are not kept on the premises. Food is brought from home and morning and afternoon snacks are provided. Bottles and food containers brought from home are properly labeled with name and date and other refrigerated items. Trash receptacles containing solid waste have tight fitting lids. Food prep areas were clean and sanitary. Food is properly stored. Menus are posted where they can be reviewed by parents. Floors, equipment, and furniture were clean and observed to be in good repair and free of sharp edges. There is drinking water available to children indoors by bottled water. Changing table has raised sides at least three inches high and is within an arm\342\200\231s reach of a hand washing sink. Toys observed to be clean age appropriate. Continue to page 2. Page 2 5 Staff files were reviewed for staff present during the facility inspection on this date. Beginning September 1, 2016, Health and Safety (H&S) 1597.622 states, a person shall not be employed or volunteer at a day care if he or she has not been immunized against influenza, pertussis, and measles. Proof of immunization against pertussis, measles for staff members were reviewed and 1 (one) of 5 (five) staff records reviewed did not have health screening or immunization records on file. 1 (one) staff stated that employee has recently been hired. Beginning March 31, 2018, H&S Code 1596.8662 requires all directors and employees to complete mandated reporting training, and to renew the training every two years. At least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications, which expires 5/31/2027. 5 Children's records were reviewed, and there was a separate, complete, and current record for each child. LPAs observed and reviewed LIC 9227 Individual Infant Sleeping Plan and 15 minute sleep checks in children\342\200\231s files. Care and supervision were evaluated to determine if the basic needs of infants are met and appropriate. Infant Needs and Assessments are done quarterly or updated as needed. In the areas reviewed the children\342\200\231s files were found to be in full compliance. Sign in/out procedure was reviewed for compliance. The person who signs the child in and out uses their full legal signature and records the time of the day. Facility uses an App called Brightwheel. Facility documents diaper changes and nap times in Bright Wheel that parent/guardian have access to. Director stated there are no children enrolled who take medication or need medication at this time but does provide services. LPAs and Director discussed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Page 4 CCC COMPLETED TESTING Lead testing on 2/20/2024 Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. Director stated that there is one water source that did not pass that is outside the fence area of the playground and has been replaced with a filtered drinking fountain source. LPA obtained led testing completed records, picture of water source, and facility sketch to review. During inspection LPA observed one staff remove a small blanket from a sleeping infant and drape it over the side rail of the crib. LPA informed Staff that there is to be nothing in the crib or on the side of the cribs while infants are sleeping in a crib. Staff stated the infant had just fallen asleep and are aware of safe sleep regulations and removed the item. The (licensee / director/ facility representative) was informed, and website given, about the California Child Care Disaster Plan has been posted to the UCSF California Childcare Health Program website: cchp.ucsf.edu/content/disaster-preparedness Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org A copy of the California Department of Social Services Lead Information Brochure was explained and provided to the facility representative. Based on LPA\342\200\231s observations, record reviews, and interviews the following violations were observed and are being cited in accordance with California Code of Regulations, Title 22, Division 12, Chapter 3. 1 Type B violation in the area of 101216(g)(1) Staff Records. See the attached LIC 809D Exit interview conducted and report was reviewed with the Director Cheyenne Cazares. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
