Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

101216.3 Teacher-Child Ratio"(b)The licensee may use teacher aides in a teacher-child ratio...(1) A ratio of one fully qualified teacher...and one aide for every 18 children in attendance in a preschool program is allowed..." This requirement was not met as evidenced by: Based on interviews and record review, on 10/25/24 at approximately 11am at the time of the incident a fully qualified aide and fully qualified teacher had 21 preschool children. This is an immediate risk to the health, safety, and personal rights risk to the children in care.

101229 Responsibility for Providing Care and Supervision"(a) The licensee shall provide care and supervision....(1) No child(ren) shall be left without the supervision of a teacher at any time..."This requirement was not met as evidenced by: Based on interviews and record review, S3 found Child 1 (C1) outside by room 6 door area. C1 did not have care and supervision in that period of time. This is an immediate risk to the health, safety, and personal rights risk to chidlren in care.
