What the inspector wrote

On 3/12/2026 at 12pm, Licensing Program Analyst (LPA) Sarah Garcia conducted an unannounced case management- incident visit in response to a self-reported unusual incident report (LIC 624) received on 2/27/2026 regarding a child being left unattended. Upon arrival, LPA met with facility representative, Briana Solorio. At, 12:15pm, LPA met with director, Jenny Vasquez. A walk through of the facility was conducted, and a census was taken. LPA observed facility during nap time and total census was 90 preschool children and 8 staff. A review of staff criminal clearance records on this date 03/12/2026 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. On 02/27/2026, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. On 2/27/26 the facility reported that Staff 1 (S1) was supervising a classroom of 11 children and during transition from indoors to the outdoor playground area. The class walked through room 10 as part of the route to the playground. During the transition Child 1 (C1), stopped inside room 10 to look at books. S1 did not complete the required face-to-name process and did not realize that C1 had remained inside the room 10. S2 was on her way to relieve S1 for a break. As S2 passed through room 10, they observed C1 unattended in the classroom. Facility determined that C1 had been alone in the classroom for approximately 7\342\200\22310 minutes. S1 was placed on administrative leave on 2/27/26. Continued on Page 2 Page 2 During today's visit, LPA interviewed 2 staff, and obtained child supervision records (CSR) for the sports room (consisting on rooms 6 and 7) on 2/27/26, C1's LIC 700, and 3/11/26 training agenda and staff sign in sheet. During staff interviews, S2 stated they were relieving S1 for a break and while walking through room 10 to the outdoor playground area they observed C1 unsupervised by the bookshelf in room 10. S3 stated that S2 notified them that C1 was found in room 10 by the bookshelf. S3 spoke with S2 who stated they may have been talking with the sport class teacher and did not see C1 come out of room 10. S3 stated that S1 did not follow proper name to face procedures of counting the children after transitioning from outdoor to indoor. S3 stated the District Manager reviewed the camera footage and confirmed that C1 was left unsupervised in room 10 while the rest of the classroom was outside. Based on LPAs interviews and record review, Child 1 (C1) was left unattended in room 10 for approximately 7-10 minutes and was not provided care and supervision during that time . According to Title 22, California Code of Regulations, Section 3, Chapter 4, 101229(a)(1) Responsibility for Providing Care and Supervision (1) Type A deficiency will be cited today. See attached LIC 809D. LPA Sarah Garcia informed director, Jenny Vasquez that this report dated 3/12/26 documents (1) Type A citation which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Sarah Garcia informed director, Jenny Vasquez to provide a copy of this licensing report dated 03/12/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview conducted with director, Jenny Vasquez. Notice of Site Visit was provided and must be posted for 30 consecutive days. End of Report

This is an immediate risk to the health and safety, and personal rights of children in care.
