Based on LPAs observations and inspection on 8/22/24, the garbage or dumpster is located outside, in the parking lot and inaccessible to children in care. Based on staff interview on 8/22/24, staff stated children does not have access in the outdoor area where the garbage or dumpster is. On 9/9/2024, LPA interviewed five (5) parents. None of the parents interviewed disclosed information that could support the allegations. Based on observation, interviews conducted and records reviews, the preponderance of evidence standard has not been met. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Exit interview was conducted and report was reviewed with Director Vanessa Stahl. Notice of Site Visit was posted during the visit. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The Director was provided with a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
