What the inspector wrote

On 11/14/25, at 10:15am, a case management incident inspection was conducted by Licensing Program Analyst (LPA) Anna Chan who met with Director Iris Antuna who was informed of the purpose of visit. Upon arrival there were 43 preschool children and 5 staff. Children were doing indoor activities when LPA arrived. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions A self-reported incident by the facility was received at the Regional Office on 11/12/25, which stated that on 11/11/25, the authorized representative of Child 1(C1) informed the facility that C1 was allegedly hit by Staff 1 (S1) on the face. LPA interviewed S1 who stated no hitting happened to C1. Staff 2 (S2) stated they were in the classroom with S1 and S2 did not witness S1 hit C1. LPA interviewed C1, C1 stated S1 did not hit C1. C1 also stated, no friend hit C1. No deficiencies cited during this visit. If any deficiencies are observed, disclosed or discovered, they will be addressed and cited at a later date. Exit interview was conducted with Director Iris Antuna. The Notice of Site Visit was provided and be posted for 30 consecutive days. End of Report
